How To Enrol

We welcome new patients residing or working in Lincoln Township and the immediate surrounding area. Enrolment is subject to our practice boundary.

You can call into the Medical Centre and ask for an enrolment pack from our reception team or click on the Enrolment form PDF below, print it and bring it back to us completed. We will then check your eligibility to enrol at Lincoln Medical Centre.
